All students enrolled in a program operated by the Distance Degrees and Programs (DDP) office are students at the Skaggs School of Pharmacy and Pharmaceutical Sciences (SSPPS), and as such, all students are subject to the policies and procedures provided in the documents listed below. In addition, students will review and are subject to the resources as listed on the Orientation webpage.
Transfer of Credit Policies
The University of Colorado mandates all students in health care professions, whose training includes clinical settings and patient contact, complete standard immunization requirements upon admission and prior to rotations. The sections below provide more information about when North American-Trained PharmD (NTPD) and International-Trained PharmD (ITPD) students should submit their immunization and certification requirements. Students should refer to the Immunization and Certification Requirement Packet for instructions on how and when to complete the requirements. In addition, print out and submit the corresponding forms and handouts at the appropriate deadlines.
Most rotation sites now require the Office of Experiential Programs to verify students have completed all their immunization and certification requirements several months prior to the start of a rotation and these requirements must remain up-to-date throughout the rotation. Therefore, students will maintain all ICs throughout their time in the program.
The forms and handouts section provides the downloadable forms that are needed to complete specific to immunizations and certification requirements listed in the Immunization and Certification Packet. Not all forms and handouts are required for every iPharmD student. Refer to the Immunization and Certification Packet to see what specific items are required for ITPD and NTPD students.
Required for all ITPD and NTPD students to complete the criminal background check. Completed Address Verification Forms are to be emailed back to Pharmacy.Online@cuanschutz.edu.
The CPIC Consent form in addition to the Address Verification Form and UC Background Disclosure Form are to be completed by iPharmD students residing in Canada only. Completed forms are to be emailed to Pharmacy.Online@cuanschutz.edu.
All students completing a site rotation in the state of Colorado are mandated by state law to complete a drug test. This handout lists locations associated with the campus and that accept walk-in as well as scheduled appointments.
The criminal background check stays current for two years. The background check is required upon admission but will need to be updated prior to rotations. All students will need to download and complete the CU Background Disclosure and Authorization Form and the Address Verification Form and email it via secure email to Pharmacy.Online@cuanschutz.edu
By signing and uploading this document to CORE the student acknowledges and agrees that they have read through the policies and procedures on the School of Pharmacy website and agrees to follow The University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences and Distance Degrees and Programs policies and procedures throughout their participation in the program. Download this document in Forms and Handouts and sign and date once all policies and procedures are fully understood. Upload the signed form to CORE to complete the requirement.
Students may apply online for their Colorado Intern License through Colorado Department of Regulatory Agencies (DORA). Use the intern license Checklist to start your application for a Colorado Pharmacist Intern License.
All PharmD students will need to download and sign The Family Educational Rights and Privacy Act (FERPA) Waiver. This signed form grants the Office of Experiential Programs and DDP Office the ability to confirm with the rotation site the student’s immunization records, background check information and drug test results are current and provide sites with this documentation if requested. Certifying officials are to fill out the Skaggs School of Pharmacy and Pharmaceutical Sciences (SSPPS) Immunization form. Refer to instructions listed on the back of the SSPPS form to be aware of what can and cannot be submitted for each immunization type. Immunization proof for the following are required: Measles, Mumps, Rubella, Hepatitis B, Polio, Tuberculin, Varicella and Influenza. Please be aware that Ministry of Health Letters are not considered acceptable forms of documentation. Upload this completed form to CORE.
In addition to the Background Disclosure and Authorization Form and the Address Verification Form, students living in countries outside the US and Canada with need to email this completed form to Pharmacy.Online@cuanschutz.edu

The University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences is accredited by Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education. There is no charge for CE credits earned while enrolled as a DDP student; however, completion of all course and instructor evaluations is required to receive CE credit. CE credit is not provided for every unit of every course, yet we try to provide enough credit so that DDP students who take at least one course per semester should be able to fulfill their CE requirements for pharmacy licensure.
The Office of Continuing Pharmacy Education posts CE credit to CPE Monitor® for students who provide an NABP e-Profile ID number and month and day of their birthdate. Credit is posted within 60 days of the end of the semester; credit cannot be posted to CPE Monitor® after 60 days due to ACPE regulations.
If you do not have an NABP number, you can apply for one online. If you are a foreign pharmacist without a US social security number, you will need to apply for an NABP account. You will have to complete the Non-United States Pharmacists Waiver and Release Form. Once you have submitted the form, NABP will create your e-Profile and provide you with the steps for completing your CPE Monitor registration. NABP Customer Service can be reached at 847-391-4406, Monday-Friday from 9 am to 5 pm Central Time, or via email. DDP students may contact the Office of Continuing Pharmacy Education for more information about CE credit for coursework or CPE Monitor®.
Graduation Ceremony
Students can graduate in May, August and December. The formal graduation ceremony is held in late May at the Anschutz Medical Campus each year. Students who graduated the previous December, August or will be eligible for the current May graduation can participate in the May graduation ceremony. In the spring semester, the DDP Office will email to all eligible students instructions on how to plan for the graduation ceremonies. Additionally, students can download a copy of the current graduation ceremony instructions, which are updated annually.
Diploma Application
All students must submit a diploma application through UCDenver Access. The deadline to submit the diploma application is located below in the semester specific graduation deadline tabs.
Students who have an unpaid balance on their account or who have not submitted their diploma application by the deadline will not receive a diploma.
Graduation Requirements and Deadlines
A 2.0 cumulative, professional grade point average (GPA) is the minimum GPA required of all graduating Doctor of Pharmacy students. Students must also receive a passing grade for all required and elective advanced pharmacy practice experiences (APPEs), which includes both rotations and credit-by-challenge submissions.
All coursework, didactic and experiential, must be completed within six contiguous calendar years. Any exception to this requires approval from the program director and the DDP Committee.
In addition, students who miss the deadlines identified below may not graduate by their anticipated graduation date.
